THE SPECIAL NEEDS CHILDREN

upon graduation, we need to learn on how to give oral hygiene education to several groups of people. my group were scheduled to give talks to the children with special needs in Klinik Kesihatn Bunohan, Tumpat; a 45-minutes drive from USM. in the clinic there is a special center where children with disability around Tumpat area gather everyday. they were taught to read, write and how to take care of themselves. it's a learning center for them.


when we arrived there, we were not quite prepared cause we are the first group who went there. so we didn't really know what to bring and what to do. our mistake was we didn't bring any LCD or laptop cause we assumed that they will prepare it for us. our mistake for expecting things so high. Tumpat is a village area, we should be prepare instead of assuming things. so we learned something there. as a solution, we opt for a traditional way.


and that is by showing pictures manually and divided each of them into groups so that we can teach them personally on how to take care of their teeth. i was in charge with a group with learning difficulties. i didn't know their actual disease, but i'm assuming some of them had autism or attention deficit or ADHD. cause they looks very normal. but had difficulty to speak and understand. other groups included children with cerebral palsy (CP) and down syndrome's children. i didn't about my friends but i found that it is very hard for me to teach children with learning disability. they didn't look at u or gave u any attention. all they were interested in were the "big teeth" model that i brought.


most of the children there never have a visit to a dentist. so u can guess how they teeth looks like. crooked anterior teeth and so.with the little knowledge that i had, i try my best to teach them. especially to this lady,who had a 2 years old son. she never cleaned his teeth. so i'm trying my best to teach her cause her son is still young, do we can still take care of the teeth.

he had a very dedicated father. this child is had CP and grind his teeth a lot. so he got lotsa teeth with attrition.


some of them came from a poor family, so they came to the center with a van. the van's owner only charge them half price. a kind man i believe, it's very hard to find a person who did things for free nowadays.


they look normal ain't they?! some of them aged more than 20


i had a lots of fun with them. looking forward for more activities like this in the future....

ART AND DENTISTRY

"Dentistry is an Art" Dr ZM.


i was scold by my supervisor that day cause my restoration was not so aesthetic. it makes the patient smiles weird and it didn't look like his natural teeth at all. i was trying to build a fracture crown to the patient and it takes me almost one hour to trim the crown so that it looks like his adjacent teeth. i can't finish my work as it was already 1230pm.


she said it with this looks that makes me think whether i'd made the right choice in learning dentistry. i never loves art. i learned it until i'm in my form 3 and i hate it and i'm not artistic either. i've a pair of hands that is worse than a wood.


i regret that i never love art. cause i now she's right. it will be easier for me to learn dentistry if i'm more creative. cause i'd seen my friends who can finish doing a cavity in less than 2 hour while for me it may takes more.


i'm not good in bending wires.
i make bad impression for patient's models.
i make hideous restoration.
i'm not gentle enough during extration.
my suturing sucks.
i can't recall anything in dentistry that i'm good at.

can i just become a housewife and stay at home?!


but it's already too late. i already dive in too deep, walk out so far, fly up damn high. it's not the time for me to look back. i need to move forward. oh Allah please gives me strength cause i know everything happens for a reason and only U who knows it.


but one advice i can give to anyone who's planning to do dentistry. u need to know how to manage your stress level. and it's an advantage if u have miracle hands.

THE DOCTORS

although i didn't consider myself a doctor, my patient still addressed me as one.

1. I gave a talk after I was conferred an honorary degree by the International Medical University. I directed a major part of my talk to the new graduates.


2. I explained that medicine is not just a profession, a qualification for earning a good income. It is a vocation, a calling which involves dedication to the job of healing the sick and caring for them.


3. What their qualification confers upon them is not just a degree but as doctors they have been elevated to a special status and endowed with special powers. They would have in them the capacity to inspire confidence and trust in their patient. The confidence and trust are such that people would literally entrust their lives to them, allowing them to cut open their bodies and do things which could kill if done by others.


4. The skill and the power they acquire owe much to the society in which they were brought up and their access to education up to the highest level. Not all human society can do this. A poor society, an unstable society, an uncaring society would not be able to give them even primary education, much less training to become a doctor. The cost borne by the society is high.Whether they get a scholarship or their parents pay for their education, they all owe a debt to society. It behoves them to repay to society through the service in which they are trained.


5. They should therefore be ready to offer their services to the society in which they lived. It may be by serving the Government or if this is not attractive enough, at least their country. They can earn a good income in Malaysia's private sector.


6. But some easily forget their debt and are easily enticed by higher pay in other countries. These countries paid nothing for their education and training and yet for a little bit more money they get the services of the people we paid a lot to train whether in Government school and universities or in private ones. Quite often the countries which get the service of our doctors are developed and rich.


7. The nation loses a lot when the people we train opts to work in other countries.


8. But the association of doctors make matters worse by refusing entry to foreign doctors to practice. It wants to keep the opportunities for making money in this country to its members only. It does not mind Malaysian doctors going out but foreign doctors may not come in. Only if they work with the Government can they come in.


9. The flow is one-way. Our doctors can leave the country but foreign doctors cannot replace them. We are losing the brains that we develop without the foreign brain coming in.


10. Despite all the Government's efforts we are losing especially the much-needed specialists.


11. There is something wrong here. If the Malaysian Medical Association (MMA) wishes to close the country to outsiders then it should also object to Malaysian doctors from leaving the country. As it is the MMA seem to be wanting to have their cake and to eating it as well.


12. I am not suggesting that as we have embraced globalisation and the free flow of capital etc that we should now allow foreign doctors to come and open their hospitals here. But I do think that if local hospitals need to employ foreign doctors then they should be allowed to.

BAD BREATH

have u ever noticed that u had bad breath or halithosis? this problem is very severe but only some people notice and care. i had encounter patients with realy really bad breath. it was so severe until i had to wear double layer of mask. or else i can't continue my work. i was breathless. seriously!
.
for me. i am very particular about my breath. yeah. i admit sometime it stink. but i try my best to  avoid it from happening. the think u can do to avoid bad breath are
  1. brushing your teeth. brush your tongue too. that is the place where bacteria always stay and make your mouth stink like longkang
  2. u can use mouth rinse
  3. eat sugarfree chewing gum
  4. avoid stinky food like garlic and petai
  5. have u ever try eating ulam like ulam raja and pucuk daun limau? try it. it makes your breath smells better.
.
if u had try all this but still your mouth is stinky. please visit the dentist. u may need scaling. some health condition like xerostomia or reduce in salivary secretion can also cause the same problem
.
for those who's their job need to be close to some one. like dentist, doctors, medical, dental student, nurse, teacher, PA and many more, u need to check your breath's smell all the time.
.
the reason i wrote this post is because one of my coleague asked me how do u approach those who had bad breath when they are your friend!!! this is very hard to do. because u  may break some one's heart. but if u didnt tell them u'll suffer smelling their bad breath everyday because they are your close friend. u see them everyday.
what u can do is you have to play with their psychology. u can start with "i think my breath stink! do u think so? how bout your breath? do u ever have problem like this?". u can even asked them to go with u to see the dentist. or u can even give them mouthwash or sugar free chewing gum as present.
there are many ways. u just need to choose the correct one.

anyway, self awareness is important. so. check your breath k!

TERMINATING PATIENT

i always feels tired nowadays.
hidup ku sgt tak productive!
.
today my patient who was supposed to come did not turn up. i call him many time. just to know that he is still sleeping. i wonder why they requested for treatment but when we wanna treat them (for free) they did not turn up. i gave him another appoinment and asked my supervisor to sign the folder to show that he failed to attend. my supervisor was quite mad. she asked me wether that patient really want treatment or i should just terminate this patient.
.
i really really wanna terminate this patient from getting treatment. but the termination procedure is menyerabutkan. so many procedures to follow before i can terminate the patient.
.
luckily i have a patient who's also a student here. he was my first patient. i asked him to come to the clinic for scaling. but he sounded scared. he's a dental student. but doesn't behave like one. when i did scaling on him, he said it was damn painful. i was mad of cause. because if i did not scale completely. i can't proceed to the next procedure. so after scaling i showed the work to my supervisor. she was not satisfied. "ado calculcus setambung, gane nak stark kijo laing nyo" bahasa kelantan mix with terengganu sikit. so my supervisor asked me to give local anesthesia, so that patient will felt no pain and i can continue my work.

of cos la patient i tak mau. so i asked him to bare with the pain if he did not want to be injected. then after sesi paksa memaksa with the patient, i completed my work and show them to the supervisor for approval. she said it's ok, but not to ok. after checking for a few minutes. then only she said that the patient had hypomineralised enamel, that was why he felt pain. because his teeth had thin surfaces.

i felt sorry for the patient because i was mad at him earlier. sorry dear patient. nasib ko student gak.

im going to rumah anak yatim tomorrow. i'll be their facilitator. it's one of the requirement that we need to fulifill before we graduate. we need to give talk on the dental health. i hope i'll enjoy

UPDATE ON MY REQUIREMENT

after 2 months i entered clinical year.


  1. i only manage to extract 10 teeth. actually 8 cos 2 were done by the surgeon.
  2. do 3 block using LA
  3. do only 7 filling. (1 class I, 2 class II, 1 class 111, 3 class V)
  4. take a lot of impression. most of them were rejected by the doctors
  5. take berpuloh-puloh x-ray yg tak jadi. only 5 x-ray were accepted by the lecturers
  6. do scaling to most of my patient
  7. examine 2 pediatric patient. (i'm supposed to examine 8 patient)
  8. do 2 examination on patient with gingivitis.
  9. do fissure sealant to 2 patient.
  10. assist 1 minor oral surgery


it looks like i'm way behind. compare to all of my friends. especially my partner who's very skillfull. she did her job well and fast. i wish i could be better than her.


i must admit that i'm lembap. huhu!!!

EXO

today i had a 16 y.o patient that request her teeth to be extracted. the tooth had caries. it can still be restore. we even offer her to do RCT. so that we can safe her tooth. but i don't know why, she still opt for extraction. it's not that there is no other option.

in my heart i was thinking that may be this patient did not had the knowledge. may be she think that there will be a new tooth to replace that tooth.

to everyone out there. i would like to make it clear that we, human are only given with 2 sets of teeth. one set is the small teeth u had during childhood. and then during adulthood u had your permanent teeth. that is why it is called as permanent tooth or gigi kekal. it is because there won't be a replacement after u lost that set of teeth. u can only replace them by denture or bridge or maybe implant. but that won't be similar to the original one.

so. please!!! take care of you teeth. if u are 16 and u already starting to extract your teeth. how many teeth will left by the time u reach 40??
